Please Read Carefully: I/we the parents of the above named child, who is a candidate for a position on a Twin Boro Bears Football Team, hereby give my/our approval for his/her participation. I/we assume all risks and hazards incidental to the conduct of the activities and transportation to and from the activities. I/ we do further hereby release, above (Twin Boro Bears) and hold harmless organizers, sponsors and supervisors, and any and all of them. In case of injury to my/our child, I/We hereby waive all claims against the organizers, sponsors and any persons transporting my/our child to and from activities. I/we understand that the organizers carry limited medical insurance and that I/we will be responsible for any and all medical coverage, if needed. I/we understand that if any injury does occur, the athlete will not be reinstated unless a note from a physician is presented to the administrator in charge.

EQUIPMENT Additionally, any and all equipment that is provided to you for participation in these activities is your responsibility, a $150 deposit will be required at equipment hand out (contact football) and will be deposited by the program should this equipment be damaged due to mistreatment or not returned within 2 weeks of the season's end or on the announced turn in date.
